I authorize 4 Paws Playhouse to do whatever they deem necessary for the health and well being of my dog(s) and agree to pay for all expenses relating to the same. I agree to pay for auxiliary services or products requested by me such as but not limited to training, transportation, bathing, etc. I agree that I am solely responsible for any damages that the above mentioned pet may cause through malicious or improper conduct to property or another boarder. 4 Paws Playhouse will not be held responsible for any loss or damage to your property including, but not limited to, your dog’s bed, blanket, or toys.

I agree to pay nightly boarding at the agreed rate. Should my dog require special care (geriatrics, etc.) I authorize 4 Paws Playhouse to provide that care and I agree to pay for those services.

I agree to pay a $20.00 cleaning fee should my dog dirty or damage the boarding room I agree to pay additional charges if I pick up my dog(s) outside of 4 Paws Playhouse posted operating hours. I understand that even for a fee, I may not be able to pick up my dog(s) outside of 4 Paws Playhouse posted hours. I also agree to contact 4 Paws Playhouse in the event I am unable to pick up my dog(s) on the date set forth above.

Please give my dog the following services while boarding at 4 Paws Playhouse:

Bath and Brush (extra charge)
Nails Cut (extra charge)
